In our continuing efforts to reduce state spending and the impact on the environment, the Employees Benefits Council’s Provider Directory is “going green.” Instead of traditional printed booklets, the lists are being presented online.

Please note the lists are by zip code and are meant as a guide. The lists represent the HMOs' current coverage areas and are subject to change. Please contact your HMO, provider or facility for confirmation.  These lists will be updated quarterly.
